[Form Applicable For]
(1) Individuals not affiliated with the School of Nutrition and Health Sciences (except faculty members co-advising students from SNHS) who want to borrow our instruments. [Click here for available instruments]

For students from the Graduate Institute of Metabolism and Obesity Sciences (GIMOS) who wish to use the autoclave, please register directly using the paper sign-up sheet. However, if faculty or students from either the SNHS or GIMOS plan to use the autoclave specifically for Agar sterilization, please make sure to fill out the online form.

[Application Procedure]
※For non-SNHS teachers or students (except co-advisors from SNHS) applying to use the equipment
(1) Please complete this form at least one week prior to the intended use date.
(2) After submitting the form, you’ll receive a confirmation email. Please forward that email to the person in charge of the equipment, and CC your advisor, our school (nutrition@tmu.edu.tw), and Assoc. Prof. Ya- Ling Chen (ylchen01@tmu.edu.tw), who leads our Property Management Committee.

(3) Once the teacher in charge approves it, you can use the equipment during your requested time. Don’t forget to log your usage properly.
(4) If you want to cancel your booking, just email our school contact (nutrition@tmu.edu.tw).

※SNHS and GIMOS intend to use the autoclave to sterilize Agar
(1) Please complete this form at least 1 day before using the equipment.
(2) After submitting the form, you will receive a confirmation email. Please forward this confirmation to both the psupervising lab(mailto: a03572@tmu.edu.tw) and the SNHS(mailto: nutrition@tmu.edu.tw).
(3) You may only proceed with paper registration and use the equipment after receiving approval from the supervising lab. Please make sure to record your equipment usage properly.

(4) If you wish to cancel your reservation, please notify the supervising lab directly at a03572@tmu.edu.tw.

[Important Notes]
(1) This form is just for applying to use the equipment. Students and teachers from our school get priority, so your booking might be canceled if the equipment is needed internally.
(2) Please read our lab rules carefully before using anything. [Click here to read]
(3) Make sure you know how to use the equipment (some items require training). If anything gets damaged due to misuse, you may need to cover the repair or replacement costs, based on our rules.
(4) You’ll need to check your booking status on your own(https://docs.google.com/spreadsheets/d/1SpswrDKFC2CIDq6ggaNfIeYkIaFv6FbUQBdbzm-faPM/edit?usp=sharing) - we won’t send out approval notices separately.
(5) If you break any of the rules, your case may be discussed in the Property Management Committee and could result in penalties.
(6) The
School of Nutrition and Health Sciences may adjust or update the usage rules at any time.
